E-Statement/E-Notice Disclosure Agreement

By accepting this document, you consent to receive via electronic delivery all required statements, notices, and disclosures, regarding all deposit accounts and lines of credit you have with State Highway Patrol Federal Credit Union. Disclosures that may be delivered to you electronically may include, but are not limited to:

 Regularly scheduled account statements

 Annual Privacy Policy

 Annual Electronic Funds Transfer Notice

 Change-in-terms information for your account

 Notices of activity on your account such as NSF items, overdraft status, returned items.

 Other disclosures we may be required to provide.

Accessing your State Highway Patrol FCU E-statements/E-notices confirms your agreement to be bound by all disclosures and agreements and acknowledges your receipt and understanding of this agreement. By entering into this agreement, you understand that State Highway Patrol FCU will no longer be providing you with a monthly paper statement. You will receive an e-mail notification that your statement(s) are available, 2-5 days after your statement cycle.

ACCESSING YOUR E-STATEMENTS

To receive electronic delivery of your statements and/or disclosures, you must have access to the following:

 A valid e-mail account capable of receiving/viewing .pdf attachments.

 A computer with Internet connection that supports 128-bit encryption.

 Adobe Acrobat Reader software (available at no charge at http://www.adobe.com)

 Microsoft Internet Explorer (version 7.0 or higher)

 Mozilla Firefox (version 3.0 or higher)

 Username and password for SHPFCU Online access.

E-MAIL ADDRESS

It is your responsibility to notify the credit union of any changes or updates to your e- mail address to avoid any delays in receiving your E-statement/E-notice. If State Highway Patrol FCU receives notification that electronic delivery is not possible due to an incorrect or closed email address, we may elect, at our discretion, to choose to revert your account(s) to standard delivery, via U.S. Mail, without additional notification.

SERVICE AVAILABILITY

The service providing E-statements/E-notices is generally available 24 hours a day, 7 days a week; however this service may be unavailable from time to time for routine software and hardware maintenance, or due to unscheduled down time.

ERROR RESOLUTION

You understand the importance of your role in preventing misuse of your account. You agree to promptly examine your statement and notify us immediately of any errors in your account. We must hear from you no later than sixty (60) days after we sent the first statement in which the problem arises. See “CONTACT METHOD” below.

CONFIDENTIALITY

You agree to protect the confidentiality of your account, account number, and your personal identification number.

CHANGE IN TERMS

It may be necessary from time to time to change the terms or conditions regarding your statement access. In the event such a change is necessary, we will display a message via e-mail notification or by written notification.

CANCELLATION OF E-STATEMENTS/E-NOTICES

You have the right to terminate your E-statements/E-notice access and receive paper statements/notices at any time upon delivery of written notice to State Highway Patrol FCU. If you wish to cancel E-statements/E-notices and resume receipt of paper statements, you must notify us by using the CONTACT METHOD below. If you cancel receipt of E- statements/E-notices and wish to resume receiving E-statements/E-notices in the future, it will be necessary for you to go through the sign-up process again. State Highway Patrol FCU has the right to terminate its obligation to provide E-statements/E-notices service to you upon ten days of prior written notice (e-mail acceptable).
